How to use this action items template
- Open it before the meeting. Fill in the meeting name, date and attendees.
- Capture items as they're agreed. Don't wait until the end, when half the room has already left.
- Name one owner. If two people share a task, pick the one who will chase it.
- Set a date. A rough date is better than none, and it can always move.
- Review it next time. Open the next meeting by walking through the tracker. Anything that slipped goes into the carried-over table with a reason.
Why action items get lost
Most action items don't fail because people are lazy. They fail because they live in meeting notes, and nobody opens meeting notes again. The fix is to move action items somewhere the team already works, like a shared board, with an owner and a date.

- Weak: Follow up on the website.
- Strong: Sam to send homepage copy changes to the designer by Thursday.
- Weak: Budget discussion.
- Strong: Priya to share a revised Q4 budget with the leadership team by 15 Oct.
